This program will help you set up a dietary supplement program designed to help you maintain your health and prevent nutritional deficiencies. You will answer a few simple questions about your lifestyle and diet. The results are for informational purposes only, and the suggested dietary supplement suggestions are not intended to treat any disease, nor have they been evaluated by the FDA for treating any disease.

Before you answer the screening questions, please keep the following points in mind:
